I have restarted OneSwarm, the problem persists.

Are you sure you didn't add it as a watch directory? Watch directories are persistent: they will keep trying to add everything in them. You may want to use the 'multiple' option in the share dialog (the one that pops up from the 'share' button)
Adding a new task is now impossible, only the old tasks are visible.
No, it's not a watch directory, was a one off "Multiple files or directories".
Aah, interesting. Re-started _again_ from the Classic UI and the zombies went away. May be something to look at anyway.
Some debug information:
1) The files are on an NFS server (2GHz-ish, 1GBps, 100MBps disk reads)
2) There were two simultaneous hashing threads running.
3) Network connectivity issues possible (large network).
